Navy Officials Cite 'Pilot Error' In 2013 Jet Crash Near Harrington

On Tuesday, the Navy's Pacific Fleet Headquarters released a nearly 400 page report explaining their findings after an EA-6B Prowler crash near Harrington, Washington last year. The crash killed 3 Navy officers including 34-year-old Lt. Cmdr. Alan Patterson of Tennessee, 24-year-old William Brown of Illinois and 26-year-old pilot Valerie Cappelaere Dalaney of Maryland. Navy officials found that several factors contributed to the crash. The jet was found to be mechanically solid and not a factor.

The Navy said Tuesday that all three officers were "technically" qualified to fly the jet, but "unnecessary" accelerated training of Lt. Cmdr. Patterson and together in the same aircraft, the three officers were not completely prepared for the low flying, high risk mission in which they were tasked. The 391 page report also states that "there were gaps in leadership, oversight and a singular mission focus that contributed to placing this crew in a situation they were technically qualified for but which exceeded their combined proficiency." It also says that no "punitive measures should be taken against any individual identified" in the report.

The crew was stationed in Whidbey Island where the Navy's Tactical Electronic Warfare squadron is stationed. The report says that the pilot training program there has not had a fatal crash for 40 years until March 11th, 2013 when this crash happened.
